The Pedestrian Repulsificator is an item that generates a spring outwards from the player's car, which will strongly push any nearby pedestrians. When more than one pedestrian is near, several springs will appear and hit all pedestrians, and the player only loses one use, no matter how many springs appeared. Trying to activate it when no pedestrians are around will have no effect, and will not spend a use.
The closer the pedestrian is to the player's car, the stronger the force of the spring. Generally, a pedestrian repulsificator is enough to kill a pedestrian, unless it's too far away, or too bulky.

To obtain it with cheats, press Ctrl+Alt+5 when in cheat mode or type COWCOW.
